The Medical Authorization and Release Forms are now online for your convenience. Please print the appropriate forms (volunteer or children/youth) for each family member, complete, sign in front of a notary public and bring by the church office at your earliest convenience. These new forms will be in force until they are revoked in writing by you. No more filling out forms each year! If any of the information changes, please complete new forms and bring them to the church office to replace the old ones.
Our ministry assistants would be happy to notarize these forms if you will be sure to sign them in their presence.
